Sephience™ (sepiapterin)

Following the Canadian journey from approval to public access

Sephience is approved and marketed in Canada for sepiapterin-responsive phenylketonuria (PKU). The next major question for families is public reimbursement. CanPKU+ is participating in that process by bringing patient and caregiver experience into reimbursement review.[1][2]

Where are we now?
Reimbursement review is underway
Health Canada's regulatory review is complete and Sephience is marketed in Canada. CanPKU+ submitted patient-group input to Canada's Drug Agency (CDA-AMC) in June 2026. In Quebec, INESSS accepted Sephience for evaluation on July 6, 2026 and currently lists the submission as under review.[8]

Public drug-plan decisions
Quebec conducts its reimbursement assessment through INESSS. Other participating public drug plans generally rely on Canada's Drug Agency (CDA-AMC) reimbursement review before later pricing and funding decisions.
Approved use
Treatment of hyperphenylalaninemia in adults and children 1 month of age and older with sepiapterin-responsive PKU, in conjunction with a phenylalanine-restricted diet.[1]
Clinical finding
In the Phase 3 APHENITY study, blood phenylalanine decreased by 63% with sepiapterin compared with 1% with placebo in the primary analysis set after 6 weeks.[4]
Canada status
Health Canada issued a Notice of Compliance on October 7, 2025. Sephience is listed as marketed in Canada from December 1, 2025.[1][2]

2025 to June 2026 | CanPKU+ gathers the community voice
CanPKU+ collected experiences and perspectives from people living with PKU and their caregivers to inform our participation in the Canadian reimbursement process.
June 2026 | CanPKU+ submits patient-group input
CanPKU+ submitted formal patient-group input to Canada's Drug Agency (CDA-AMC), bringing community priorities into the reimbursement review.
July 6, 2026 | Quebec reimbursement submission accepted
INESSS accepted Sephience for a first reimbursement evaluation in Quebec. INESSS lists the evaluation as underway from July 13, 2026.[8]
Current stage | Reimbursement review
The Canadian access journey is now focused on reimbursement. CanPKU+ will continue to follow recommendations, negotiations and public drug-plan decisions as they become available.
CanPKU+ Advocacy: Bringing the PKU community into the process

Access decisions are not only about clinical trial results. Reimbursement reviewers also need to understand what living with PKU looks like in everyday life, what current treatment requires, and what outcomes matter to patients and families.

CanPKU+ gathered patient and caregiver experiences and submitted formal patient-group input to CDA-AMC in June 2026. Our role was to help ensure that the reimbursement discussion included the lived experience of PKU alongside clinical and economic evidence.

1. Listen
Gather the experiences, priorities and treatment realities of people living with PKU and their caregivers.
2. Translate
Turn community experience into evidence that can be used within formal health-technology and reimbursement review.
3. Advocate
Bring those priorities directly into the decision-making process and continue following the treatment through later access stages.
Why we show the journey
We frequently ask our community to complete surveys, share experiences and contribute to advocacy. Following the treatment-access journey helps close the loop by showing where those voices go and how they contribute to the process.

As recommendations and funding decisions are released, CanPKU+ will update this page so the community can see what happens next.

Clinical evidence: What did the research show?

The key Phase 3 evidence comes from APHENITY, an international, randomized, double-blind, placebo-controlled study evaluating sepiapterin in children and adults with PKU.[4]

Study design
Phase 3, randomized, double-blind and placebo-controlled, conducted across 34 sites globally.
Participants
156 participants were evaluated in Part 1. Of these, 114 were identified as sepiapterin-responsive.
Primary result
After 6 weeks, blood phenylalanine decreased by 63% with sepiapterin compared with 1% with placebo in the primary analysis set.
Longer-term evidence
Extension data have examined durability of phenylalanine reduction and changes in dietary phenylalanine tolerance over time.

Health Canada concluded that the overall benefit-harm-uncertainty profile was favourable for the approved indication.[3]

Access in Canada: What happens next?
Health Canada approval is complete
Sephience can be marketed in Canada. This is an important milestone, but it does not automatically mean that provincial and territorial public drug plans will pay for it.

Reimbursement review

Health-technology assessment organizations evaluate clinical evidence, economic evidence and input from patients and clinicians before making reimbursement recommendations. CanPKU+ has participated in this stage through its patient-group submission.

Quebec

Quebec conducts its assessment through INESSS. Sephience was accepted for evaluation on July 6, 2026 and is currently listed as under review.[8]

After a reimbursement recommendation

A positive recommendation would still not mean immediate universal public coverage. Depending on the jurisdiction and process, further steps can include pricing negotiations, decisions by individual public drug plans, development of coverage criteria and local implementation.

What CanPKU+ will be watching
We will follow reimbursement recommendations, any subsequent negotiation stage, provincial and territorial funding decisions, eligibility criteria and practical barriers that may affect equitable access.

If you have questions about your individual treatment options or current access to Sephience, speak with your metabolic clinic. PTC Therapeutics also operates a Canadian patient support program that includes reimbursement assistance.[9]

Why this matters for Canadians living with PKU

PKU is lifelong. Treatment can involve careful dietary management, medical foods or formula, regular blood monitoring and medication. The burden and priorities of treatment are different for every individual and family.

A new treatment option is therefore about more than whether a medicine receives regulatory approval. For families, the practical questions are also: Who may benefit? Who will be eligible? Will public plans cover it? How long will access take? Will coverage be equitable across Canada?

That is why CanPKU+ follows the treatment journey beyond Health Canada approval and why patient participation in reimbursement decisions matters.

This page is an educational and advocacy resource. It does not provide medical advice and should not replace individualized guidance from your metabolic specialist or care team.
